Sciatica vs. Lower Back Pain: How to Tell the Difference

Lower back pain is one of the most common reasons people seek physical therapy, but not all back pain is the same. Understanding whether you’re dealing with general lower back pain or sciatica can help you get the right treatment sooner.

What Is Lower Back Pain?

Lower back pain is typically felt in the muscles, joints, or ligaments of the lumbar spine. It may develop after lifting something heavy, sitting for long periods, poor posture, or muscle strain. The pain is usually localized to the lower back and may feel stiff, achy, or sharp with certain movements.

What Is Sciatica?

Sciatica occurs when the sciatic nerve becomes irritated or compressed, often due to a herniated disc, spinal stenosis, or other conditions affecting the lower spine. Unlike general back pain, sciatica usually causes pain that radiates from the lower back into the buttock and down one leg. Some people also experience numbness, tingling, or weakness in the leg or foot.

Key Differences

  • Lower Back Pain: Pain stays primarily in the lower back.
  • Sciatica: Pain travels down one leg, often below the knee.
  • Lower Back Pain: Muscle stiffness and soreness are common.
  • Sciatica: Burning, shooting, or electric-like pain is more typical.
  • Sciatica: May include numbness, tingling, or muscle weakness.

When to Seek Help

If your pain lasts more than a few days, interferes with daily activities, or includes leg weakness, numbness, or changes in bowel or bladder function, it’s important to seek medical evaluation promptly.

How Physical Therapy Can Help

A physical therapist can identify the true source of your pain and develop a personalized treatment plan. Manual therapy, targeted exercises, posture correction, and movement training can reduce pain, improve mobility, and help prevent future episodes. In some cases, advanced treatments such as Summus Medical Laser (Class IV laser therapy) may also help decrease inflammation and support healing.

At Transitions Physical Therapy & Wellness Center, the team focuses on treating the root cause — not just the symptoms — so you can get back to the activities you enjoy with confidence.
