Warning Signs of Bone Stress Injuries
If you’ve ever felt a deep, nagging ache in your foot, hip, or shin that won’t quite go away—even with rest—it could be more than just overuse. You might be experiencing a bone stress injury.
These injuries happen when bone is repeatedly stressed without enough time to repair. Over time, this can lead to micro-damage and eventually a full-blown stress fracture. Picture a paperclip being bent over and over—it eventually breaks. Our bones react the same way if we don’t give them time to recover between activities.
The Numbers Don’t Lie – Annual Incidence in Runners & Athletes
- Among NCAA Division I long-distance runners, the incidence of bone stress injuries (BSIs) is approximately 32 per 100 athlete-years—with women experiencing ~45 BSIs and men ~20 BSIs per 100 athlete-years (SAGE Journals).
- In Pac-12 collegiate cross-country runners, BSIs account for around 20% of all injuries. The injury rate averages 0.14 BSIs per athlete-season, with higher rates in women (0.16) compared to men (0.10) (PubMed, PMC).
- Across athletes and military recruits in the U.S., annual stress fracture incidence ranges from 5% to 30%, depending on sport, sex, and training volume (Wikipedia).
5 Key Things to Keep in Mind
- Too much too soon – Rapid increases in activity are a top cause of running injuries and bone stress.
- Form matters – Biomechanical issues, like muscle imbalances or poor gait, increase stress on certain bones.
- Nutrition counts – Inadequate fuel or low calcium/vitamin D levels reduce bone strength.
- Hormonal changes – Especially post-menopause, bone density naturally declines.
- Pain is a signal – Pain that lingers or worsens—even at rest—should not be ignored.
The good news? Most bone stress injuries are preventable with smart training, proper nutrition, and the guidance of a physical therapy professional.
